By signing this form, I understand that this is in no way a guarantee of the organization's acceptance of my application as a vendor. I hereby acknowledge my compliance with the rules and regulations of the organization, including the cleanliness and maintenance of my surroundings as well as the premises and my conduct towards others.
I understand that I shall be liable for my own liability insurance (if applicable) and the organization shall NOT be responsible for any damage, injuries, cost, loss, and/or death to our person or property that may be incurred during the event.
The vendor fee is $100 and must be paid in advance to secure your table. Payments can be made through your local Church Administrator or via paypal. Payments should be made by June 1, 2026.
